Recorded the database and PHP in the Servers and DNS document

Internal: deployment/SERVERS.md records that the app's MySQL database is on the app server and that PHP runs inside Apache, as Apache's PHP module. The last step for people's addresses in deployment/README.md no longer reloads PHP after PEOPLE_DOMAIN is set: config.php reads .env on every request.
