Appointments setup steps use consistent spacing and buttons

Each Appointments setup step starts with a heading and description, and ends with one row of buttons. On Staff and Availability, Save and continue saves the step and opens the next one; the separate continue links were removed. On Publish, Booking page settings is a button beside the publish button.
