Internal documentation. Added docs/plans/TEAM_ACCESS.md. It records how access works today (a team membership opens every area), the decisions made on 11 October 2026, one access check asked where requests enter, the changes to invitations, the member limit and leave, and the order of work. No product behaviour changed.
